Silent Witness Form
See Something. Say Something. Do Something.
Your willingness to report suspicious behavior, policy violations, and/or crimes is essential to keeping our campus safe. We understand that for some witnessing a crime can be very distressing and generate feelings of apprehension and fear about reporting. Having the option to report crimes anonymously may alleviate such feelings. If you have witnessed a crime, policy violation, or any situation that you would like to report, please complete the form below.
Upon submitting this form, an email is sent directly to Campus Safety, which may not be viewed immediately. This form should be used to share information regarding possible criminal/suspicious activity, which includes illegal drugs or illegal use of prescription medications, weapons, theft, underage drinking, assault, sexual assault, burglary, etc.
This form IS NOT intended for situations that are currently in progress. If you are currently witnessing a crime, call 562.907.4211 or 562.907.4911 (emergencies only).
As a witness to a crime or incident, your input may potentially provide Campus Safety with vital information needed to move ahead on a case or to prevent a crime. So whether you saw it, heard it, or merely heard about it, you can help us keep the campus safe by telling us.
Please note: Although you can use this form to report a sexual assault, without your identifying information or that of the victim/survivor, the Office of Title IX may not have enough information to address the situation you have described. Appointments can be made with the Office of Title IX to discuss potential reports and identifying information does not need to be shared at these meetings.
Thank you for taking the time to provide us with this valuable information.
